I am in the process of 'form filling' I know you've all been there and done that to death... so I won't bother you with all of the questions I really have - I just need to know this:-


when filling out a form - it askes for Birth Name, first name, second name.... do they really want my maiden name - or do they mean surname - Kavanagh - then Christine Ann for the other two...


told you it was a stupid question...

Wouldn't they ask for "name de jeune fille" if they wanted the maiden name ?

And generally all govt letters will come addressed to your maiden name!
