Flew Lufthansa Frankfurt to Joburg as late as 1997 and they still had a smoking section. The smell spread throughout the plane - eleven really unpleasant hours.
Yes I recall smoking on an American Airlines 747 to Miami in the late 90s.
Oh yes, lovely, and just a step or two to walk in and during the flight a little wander up to the front where the driver would give you a go at the controls.
