Have you tried him with a soft one rather than the hard plastic? We use a soft one for our dogs.
Good to find out he has ear pain as that needs sorting.
I think you are so right not to rush to another op/biopsy. We’ve made those decisions with our elderly dogs in the past. It’s about quality of life. There are times when it’s important to be able to say “enough”.
My computer screen has loads of smudges and is wet from the stroking and kisses I’ve given to pussycat.
Now it’s time to get the wipes to clean it up. 
I’m going to try a more comfortable cat collar from Amazon. The vet’s collar is uncomfortable hard rigid plastic.

With a huge help from my lovely neighbour, and a cat association, all kitties and parents were sterilised 
The 4 kittens went to lovely homes, Mumsy, Rusty (Daddy?) Miss Kitty and Tabs stay with me.

He’s wearing his new collarette now and seems OK with it. Bought the middle size option but was too small so bought a couple more L size. He can sleep more comfortably now which is a relief for both of us!

That is so true - but we’re all sure he is loved to bits from you and, of course, there are loads of virtual friends who love him too! Glad that he’s happier with his new collarette.

It’s very nice the cats join for a walk and I see the same thing with our neighbour who walks the hills with 2 or 3 dogs and a couple of cats following…

That reminds me of a story I heard of a couple, friends of friends, who were driving back from a caravanning trip. They’d been letting the cats out while they were away so they let them out at the motorway services and the cats disappeared. They had to stay there for two nights before the cats came back.
Ummmm..
a couple of years ago, Bubbles, the black one went missing for about 24hrs and it was in the middle of summer (40 deg etc).
He came back and I guess he just go hot and found a shady place to hang out.
So, in short yes, they hang out for hours outside or inside and I call them in and they come trotting back no worries.
We are about 4 or 5 kms from the nearest tarmac road so apart from local wildlife, there are no nasty humanoid perils for them.
Must be lovely and when they are out, we say that they are in the ‘ultimate snack-bar at the end of the universe’ (after Douglas Adams)….and they do indeed snack very well on the local rodents and lizards and so on….
We used to get one of our cats - Patsy - back by shaking a box of Twiglets. 
She’d come shooting out of the far distance.
